Japan Particle-Beam Therapy Clinical Training Visa for Foreign Doctors
JAPAN PARTICLE-BEAM THERAPY / CLINICAL TRAINING
Japan Particle-Beam Therapy Clinical Training Visa for Foreign Medical Professionals
Are you a hospital, university or overseas medical institution arranging clinical training involving proton or heavy-ion beam therapy in Japan? A special period-of-stay category exists under Japan's Trainee (研修) residence status for certain overseas-qualified medical professionals and collaborating medical radiation physics specialists. Immigration permission and authorization to perform clinical procedures are separate matters. We help review the immigration route, COE paperwork and coordination with the Japanese host.

Proton and heavy-ion therapy
Check whether the proposed program concerns the specified medical irradiation equipment.
MHLW authorization
Identify clinical training permissions, designated facilities and supervision.
Special Trainee category
An eligible applicant may be considered for a two-year period of stay.
Who Is This Guidance For?
Who May Qualify for the Special Trainee Period?
The Immigration Services Agency (ISA) lists standard periods of one year, six months or three months for Trainee status. It also lists two years, one year, six months or three months for a narrowly defined category of particle-beam therapy training. The equipment must relate to medical proton beam or heavy-ion beam irradiation.
| Possible applicant | Requirements to assess |
|---|---|
| Overseas-qualified physicians, nurses and radiological technologists | Relevant qualification equivalent to the Japanese profession and MHLW permission under the foreign clinical training special legislation to acquire skills using the specified equipment. |
| Medical radiation physics / engineering specialists | Specialized knowledge of physics or engineering concerning medical radiation and a training program alongside the above authorized foreign medical professionals. |
| Dispatch requirement | The training must be part of the duties assigned by an organization with which the applicant is affiliated in their country of nationality or residence. |
Two Different Legal Processes
1. Immigration residence status
ISA assesses the intended activities and the appropriate residence status, such as Trainee. Overseas applicants may require a Certificate of Eligibility (COE), a visa and admission at the port of entry.
2. Permission for clinical practice
Foreign medical professionals who would perform regulated clinical acts without the relevant Japanese license must consider the MHLW clinical training permission framework, designated hospitals and direct supervision.
Observation-only and physics training
Lectures, observation and equipment simulations should be distinguished from procedures involving patients. Observation does not automatically qualify a person for the special two-year immigration category; nor may a medical physics specialist perform regulated clinical acts solely because they are included in the immigration description.

COE Documents and MHLW Paperwork
For this special Trainee category, ISA asks for the standard Trainee evidence plus a document proving the assignment is part of the overseas affiliated institution's duties. MHLW uses a separate set of forms for clinical training permission and institution designation.
| Workstream | Typical evidence or review |
|---|---|
| ISA: trainee and program | COE form, photo, invitation explanation, training schedule, trainee treatment summary and professional resume. |
| ISA: overseas institution | Dispatch / return-to-work evidence, sending-organization details and the additional official-duty dispatch document. |
| ISA: Japanese host | Host organization documents, supervisor resume and details of the planned instruction and facilities. |
| MHLW: clinical training permission | Overseas credentials, relevant clinical experience, agreed training plan, host acceptance and other documents prescribed for the individual's permit. |
| MHLW: facility designation | Verify existing designation or any application required for the proposed clinical training setting. |
These are planning categories, not a substitute for official document checklists. Foreign-language submissions to ISA generally require Japanese translation. MHLW forms and required evidence vary by profession and application type.
How to Arrange the Program
Identify the participant's actual role
Confirm whether the person is an overseas physician, nurse, radiological technologist or medical physics specialist, and whether direct patient care is planned.
Check the Japanese receiving facility
Review its status under MHLW clinical training rules, designated supervisors, the scope of procedures and the training program.
Collect overseas dispatch evidence
Coordinate professional qualifications, employment records, an institutional assignment letter, anticipated duties after return and proposed dates.
Align MHLW and immigration applications
Check separate permit requirements and applicable timelines, then prepare the appropriate Japan-side COE application through a legally permitted route.
Visa, entry and authorized training
After the applicable approvals, visa issuance and entry examination, conduct only those activities permitted at the relevant facility under proper supervision.
Professional Fees
| Service | Professional fee (tax included) |
|---|---|
| Standard published starting fee for Trainee COE application | From ¥88,000 |
| Specialist particle-beam therapy / multi-participant case | Individual quotation |
| MHLW clinical training permits or hospital designation | Availability, scope and fees confirmed separately |
The ¥88,000 starting fee is our published standard Trainee COE rate, not a fixed quote for specialized medical training. ISA does not charge an application fee for the COE request itself. Any applicable MHLW fees, translation, document authentication and shipping are separate.
Frequently Asked Questions
Can a foreign doctor treat patients in Japan with a Trainee visa?
Trainee residence status alone does not authorize regulated clinical practice. Relevant MHLW permission, an appropriately designated institution and supervised activities under the special legislation must be checked.
Does the special category guarantee a two-year visa?
No. Two years is one available period under a narrowly defined ISA category, not an entitlement. The approved period and the duration of MHLW clinical training permission are separate.
Can a foreign nurse undergo two years of clinical training?
Immigration periods and MHLW permit validity are governed by different rules. Clinical training permissions for nurses and certain other medical professionals have a different statutory time limit, so the profession-specific position must be verified.
Does a medical physicist require a physician's clinical training permit?
Not automatically. The immigration category can cover certain medical radiation physics specialists training with qualifying foreign medical professionals. Whether any separate authorization is necessary depends on their actual activities, especially contact with patients.
Does the two-year category cover all radiotherapy equipment?
No. The specific wording concerns equipment using proton or heavy-ion beams for medical treatment. Other radiotherapy training must be assessed on its own facts.
Is a dispatch letter from our overseas hospital required?
ISA lists an additional document proving the applicant is sent as part of the duties of an organization in their country of nationality or residence, alongside the usual Trainee evidence.
Can a hospital apply for MHLW permission and a COE together?
They are different procedures. Sequencing, required approvals and submission methods should be coordinated with the Japanese host and responsible authorities before confirming a start date.
Can your office coordinate with our overseas university?
Yes. Overseas consultation and initial documentation review are available. We will confirm the scope of permitted application assistance and the required original documents for each procedure.
